Gentlemen:
This refers to the Special Team Inspection conducted by Messrs. H. Chaney,
R. Evans, and D. Powers of this office during the period February 16-19, 1988,
of activities authorized by NRC Operating Licenses R-25 and R-126 for the
University of Utah's AGN-201 and TRIGA reactors, respectively, and to the
discussion of our findings with Messrs. R. E. Stephenson and G. M. Sandquist,
and other members of your staff at the conclusion of the inspection.
Areas examined during the inspection involved reactor operation, emergency
planning, physical security, and radiological controls associated with the
TRIGA reactor, and the physical security and decommissioning status of the
AGN-201 reactor. Within these areas, the inspection consisted of selective
examination of procedures and representative records, interviews with
personnel, and observations by the NRC inspectors.
The inspection findings are
documented in the enclosed inspection report.
During this inspection, it was found that certain of your activities appeared
to be in violation of NRC requirements.
Also curing this inspection, it was
found that certain of your activities appeared to deviate from commitments made
to the NRC.
As discussed in our telephone conversation of February 24, 1988,
between Mr. R. E. Hall of this office and yourself, we have scheduled an
enforcement conference in our office at 10 a.m. on March 28, 1988, to discuss
your understanding of these apparent violations, the reasons for occurrence,
and your corrective actions.
We will also discuss any potential enforcement
options available to the NRC, if appropriate.
Should you have any questions concerning this inspection, we will be pleased to
discuss them with you.
