During the shutdown, while attempting to place the B loop of residual heat removal into shutdown cooling, the residual heat removal system injection valve, 3-MOV-744B, did not open when demanded. The licensee then placed the A loop of RHR in service and began an investigation to determine the cause of the failure. The initial results of the investigation identified that the motor had failed. However, at the end of the inspection period, additional inspection was needed to resolve this issue.
