A reactive safety inspection was conducted on February 18-20, 1987, to examine

the events which lead to an extremity exposure to one worker in excess of

federal _ regulatory limits. The inspector also reviewed your immediate correc-

tive actions to prevent recurrence of a similar event.

Within the scope of this inspection, it appears that some of your activities

were not conducted in full compliance with regulatory requirements. Breakdowns

in your Radiation Protection Program and work practices directly contributed to

the extremity overexposure. Apparent violations are discussed in the enclosed

report. We have scheduled an Enforcement Conference in the Region I office for

March 25, 1987, at 1:00 p.m.
