The following information is required per 1 OCFR 21.21 ( d) ( 4):

It has been identified that Nutherm International, Inc has supplied Eaton relays to the following facilities:

Exelon Generation Company, LLC-Dresden Nuclear Station Exelon Generation Company, LLC - Quad Cities Nuclear Station Energy Operations, Inc. - Palisades Nuclear Power Plant Duke Energy - HB Robinson Steam Electric Plant NextEra Energy Point Beach, LLC - Point Beach Nuclear Plant iii.

Identification-of the firm constructing the facility or supplying the basic component which fails to comply or contains a defect.

• (9 09/25/2020 4:55 PM 14154847068 13018165151 pg 2 of 4 Page2 September 25, 2020 On July 23, 2019, Eaton Corporation released a letter, "Update on Reports of Size 1 & 2 AC Contactors that Failed to Open When Deenergized". This letter described a condition observed on AC contactors after being energized for a long period of time without being "cycled". Eaton also stated that this would occur in only a few isolated cases in continuously energized applications.

The cause of the problem is the coating on each of the steel laminations in the cores of these devices.

This steel is coatoo with an organic insulating varnish classified as C3. This coating, when heated for a long time period, can migrate from the lamination surfaces to the edges at the area between the separate core pieces (referred to as annatures and magnets); This substance is sticky and can potentially hold the core pieces together with enough strength to overcome the forces of the device's return springs.

Potentially impacted Eaton components may include:

Freedom contactors NEMA Size 00 through 5 A200 contactors NEMA Size 00 through 4 D15 Relays D26Relays AR Relays (AC Only)

BF Relays (AC Only and including NBF relays)

Vacuum contactors NEMA Size 4

Nutherm has provided relays to the following plants. Nutherm cannot determine if these relays are normally energized.

Entergy - Palisades Nuclear Power Plant Part Number: D26MR22A All installed in Motor Control Centers.

Exelon Generation Company. LLC - Dresden Station Part Number: AR660AR Exelon Generation Company. LLC - Quad Cities Nuclear Station Part Number: AR660AR Part Number: AR440AR

The manufacturer has completed corrective actions on A200 contactors. This was completed in the

The impacted customers were notified of this condition date September 28, 2020.

Any advice related to the defect or failure to comply about the facility, activity or basic component that has been, is being or will be given to purchasers or licensees.

The manufacturer conservatively estimated that a monthly operation ( cycling) would be sufficient to prevent the substance from binding the surfaces together. Cycling or replacement of the units in continuously energized conditions is suggested.
