Non-emergency incidents as used in this procedure are incidents that are not Unusual Events which may require external agencies to respond to the site, and/or may be reportable to Federal, State and local authorities.
2.2      For other incidents the scope of this procedure begins at the point that an incident has already been assessed using ISFSI Procedure EO-5 and found to not meet the criteria of an Unusual Event. In this case, this procedure provides direction to the ISFSI Shift Supervisor (ISS) for the continuing response to, classification, and reporting of the incident.
2.0 SCOPE 2.1 For a Medical Emergency or a Spill of a Hazardous Material this procedure is used to provide detailed mitigation instructions, and then to classify and report the event.2.2 For other incidents the scope of this procedure begins at the point that an incident has already been assessed using ISFSI Procedure EO-5 and found to not meet the criteria of an Unusual Event. In this case, this procedure provides direction to the ISFSI Shift Supervisor (ISS) for the continuing response to, classification, and reporting of the incident.3.0 DEFINITIONS 3.1 Emergency Response Organization (ERO) consists of on-shift personnel at the ISFSI, and other personnel who are called in response to declared emergencies.

6.2 Apply any applicable operational or security compensatory measures.6.3 OBTAIN the information needed to provide for the assessment and reporting of the incident and CLASSIFY the incident in accordance with Section 1 of Attachment 1.6.4 NOTIFY the ISFSI Manager (or designee) of the nature and location of the incident.6.5 PERFORM notification in accordance with Section 2 of Attachment 1.6.6 COMPLETE a Condition Report in accordance with AD-7, Corrective Actions Program.6.7 PROVIDE copies of all notifications to the ISFSI Manager or designee who is responsible for any follow-up written reports to state and federal agencies in accordance with AD- 11, "ISFSI Reportability Determinations." 6.8 Surveillances.

ISFSI Emergency Operating EO-6 Rev. 12 Non-Emergency Event Assessment 1.0 PURPOSE This procedure provides guidance for responding to, classifying and reporting non-emergency incidents at the ISFSI in an accurate manner. Non-emergency incidents as used in this procedure are incidents that are not Unusual Events which may require external agencies to respond to the site, and/or may be reportable to Federal, State and local authorities.

2.0 SCOPE 2.1 For a Medical Emergency or a Spill of a Hazardous Material this procedure is used to provide detailed mitigation instructions, and then to classify and report the event.

2.2 For other incidents the scope of this procedure begins at the point that an incident has already been assessed using ISFSI Procedure EO-5 and found to not meet the criteria of an Unusual Event. In this case, this procedure provides direction to the ISFSI Shift Supervisor (ISS) for the continuing response to, classification, and reporting of the incident.

3.0 DEFINITIONS 3.1 Emergency Response Organization (ERO) consists of on-shift personnel at the ISFSI, and other personnel who are called in response to declared emergencies. The ERO, and the administrative measures to establish and maintain the emergency response capability, are described in ISFSI Procedure EO-1.

3.2 Emergency Spill - defined as a chemical release that (1) poses a threat to human health or the environment, (2) a potential exposure condition to a hazardous material greater than the Permissible Exposure Limit for that material, or a fire or explosion hazard, or (3) exceeds established Reportable Quantity thresholds.

3.3 Incidental Spill - defined as a chemical release which does not present an environmental, safety or health hazard, and can be controlled or absorbed at the time of the spill, using normally available resources and protective equipment.

3.4 Incident - the occurrence of a potentially serious event that requires timely assessment and an appropriate timely response. Based on the assessment, notification to state and NRC officials may be required, and a response by the Emergency Response Organization may be required.

ISFSI Emergency Operating EO-6 Rev. 12 3.5 OSHA (Occupational Safety & Health Act) Notification - Work-related incidents resulting in the death of an employee or the hospitalization (in-patient) of three or more employees must be verbally reported to OSHA within eight hours of the incident. If an incident does not initially meet the reporting criteria, but, within 30 days, does meet the reporting criteria, then OSHA must be notified within eight hours of obtaining the information.

3.6 Spills to Receiving Water - Spills originating from the station that produce a visible sheen on receiving water. Receiving water refers to the Connecticut River, Salmon River and discharge canal. Oil sheens produced after rainstorms from normal and routine operations are not considered spill events.

3.7 First Aid Responder - A First Aid Responder is an individual trained in basic first aid. The ISFSI Manager may designate ISFSI Staff personnel as First Aid Responders 3.8 Medical Responder - A Medical Responder is any individual that is certified by a recognized medical organization to be a first responder to a medical emergency.

Typically a Medical Responder will be a member of the Fire Department or a member of an Ambulance Team.

4.14 ES-1, "ISFSI Safety and Health Program" 5.0 RESPONSIBILITIES 5.1 The ISFSI Manager (or designee) has the overall responsibility for the appropriate response to incidents as follows:

5.1.1 Ensure the ISFSI Shift Supervisor (ISS) is adequately trained to assess the incident and direct the incident response as appropriate in accordance with this procedure.

5.1.2 Ensure that adequate personnel and equipment resources are available to implement the actions of this procedure.

5.1.3 Assist the ISS during the implementation of this procedure as requested.

5.1.4 The ISFSI Manager or designee is responsible for any follow-up written reports to state and federal agencies.

5.2 The ISFSI Shift Supervisor (ISS) is responsible for performing the actions specified for the ISS in this procedure, including the following:

5.2.1 Coordinate the overall response to a Medical Emergency or a Spill of a Hazardous Material.

5.2.2 Perform the initial and ongoing assessment and notifications.

5.2.3 Take command and control of the ERO and contacting additional personnel as necessary.

5.3 ERO personnel shall be responsible for performing the actions as directed by the ISS.

5.4 The ISFSI Security Force reports to the ISS and is responsible for the following:

5.4.1 Maintaining facility security in accordance with the Security Plan.

5.4.2 Respond to medical emergencies at the ISFSI as a First Aid Responder, when required.

5.4.3 Respond to a Spill of a Hazardous Material at the ISFSI as a First Responder,

  • and take defensive actions when required.

5.4.4 Providing accountability of personnel at the ISFSI.

ISFSI Emergency Operating EO-6 Rev. 12 5.4.5 Ensuring/facilitating the evacuation and relocation of site personnel as directed by the ISS.

5.5 Other personnel that are available are responsible for conducting initial actions to stabilize the incident, including any necessary corrective actions or on-site protective actions, dose assessments, first aid and limited fire fighting, as directed by CY Management.

6.0 PROCEDURE 6.1 When notified of an incident at the ISFSI, the ISS shall PERFORM following:

6.1.1 If it is a medical emergency REFER to Attachment 2.

6.1.2 If the incident is an Oil Spill, Chemical or Hazardous Substance Spill, REFER to Attachment 3.

6.1.3 For all other incidents, and the incident does not qualify as an emergency event in accordance with EO-5, "ISFSI Emergency Plan Implementation,"

CONTINUE with this procedure.

6.2 Apply any applicable operational or security compensatory measures.

6.3 OBTAIN the information needed to provide for the assessment and reporting of the incident and CLASSIFY the incident in accordance with Section 1 of Attachment 1.

6.4 NOTIFY the ISFSI Manager (or designee) of the nature and location of the incident.

6.5 PERFORM notification in accordance with Section 2 of Attachment 1.

6.6 COMPLETE a Condition Report in accordance with AD-7, Corrective Actions Program.

6.7 PROVIDE copies of all notifications to the ISFSI Manager or designee who is responsible for any follow-up written reports to state and federal agencies in accordance with AD- 11, "ISFSI Reportability Determinations."

6.8 Surveillances.

6.8.1 INSPECT spill control equipment once per month for spill response material capabilities and inventory, and document on Attachment 6.

SECTION 1.0 Classification and Reporting Requirements for Non-Emergency Events (NRC and State Notifications are Required for each event on the following list)

The NRC classification for all Non-Emergency Events (i.e., any incident that does not rise to the level of an Unusual Event) is "none." The required timetable for NRC reporting requirements for Non-Emergency Events is contained in the following list.

The Non-Emergency Events contained in the following list are designated as 1-hour, 4-hour, 8 hour9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />, or 24-hour reports in accordance with NRC regulations. State regulations require notification of I -

hour, 4-hour, 8-hour and 24-hour reports within one hour of the event occurrence, as is reflected in the following list. These events shall therefore be reported to the NRC immediately after notifying the State and within 1-hour, regardless of the NRC reporting criteria time limits.

The State of Connecticut Radiological Emergency Response Plan (RERP) requires that incidents be assigned a posture code at the time of classification. It also defines two classifications for non-emergency incidents: "General Interest Events" and "Radioactive Material Incidents". The "Posture Code" and "Classification" are on the attached list for each type of incident.
