Walkdown Rqpqrt On March 12,2012, the Nuclear Regulatory Commission (NRC) issued a letter titled,"Request for Information Pursuant to Titfe 10 of the Code of Federal Regulaffons50.54(f) Regarding Recommendations 2.1,2.3, and 9.3 of the Near-Term Task Force'Review of Insights frorn the Fukushima Dai-lchi Accident,o to all power reactor licenseesand holders of construction permits in ac'tive or defened status. Enclosure 3 of the10 CFR 50.54(0 letter contains specific Requested Actions, Requested Information, andRequired Responses associated with Recommendation 2.3 for Seismic Walkdowns.
By fetter dated November 27,2012 (ADAMS Package Accession No. ML130080030),FENOC submitted the 180-day response to the 10 CFR 50.54(0 letter. The required seismic walkdown reports were provlded for Beaver Valley Power Station, Unit No. 1;Beaver Valley Powei Station, Unit No. 2; Davis-Besse Nuclear Power Station; and Perry Nuclear Power Plant. Followlng incorporation into ADAMS, thg document pages were observed to not be in the same order as the November 27,2012 submittal, and FENOC resubmitted the enclosures in elec'tronic format by letter dated April 29, 2013.During the seismic walkdowns, certaih plant areas were inaccessible due to conditionsexpected during normal power operatlon.
FENOC commifted to submit a supplementalreport documenting the results of the additional seismic wafkdowns performed during a specified refueling outage. As committed for the Perry Nuclear Power Plant, the additional seismiC wakdowns were completed during the spring 2013 refueling outage.FENOC hereby submits the enclosed addendum to the Perry Nuclear Power PlantNear-Term Task Force Recommendation 2.3 Seismic Walkdown Report.

Purpose: In response to the NRC Letter 50.54(0 Section 2.3, First Energy Nuclear Operating Company (FENOC) submitted its response to the NRC in November 2012 for its PerryNuclear Power Plant. Page 124 of the submittal report Table 6-3 identified 10components that were not accessible during the time walkdowns were performed while the plant was in operation.
A commitment was added in the report stating that these 10items will be walked down during the next refueling outage.The purpose of this addendum to the report is to document the findings of those 10components that were walked down during March/ApiI2013 refueling outage.
Attachments:1. Revised pages 61 and 66 of Table 4-L to correctly identify the building, elevationand room number for Components 1H22P0011 and 1H51P1335.
: 2. Revised pages 106 and 107 of Table 4-2 to correctly identify the building,elevation and room number Components 1H22P001 1 and 1H5 lP 1 335 .3. Revised page 114 of Table 4-3 to correctly identify the building for Component 0G41F0581.

Revised page 124 for Tables 6-2 and 6-3 to include the additional rooms walked down (DW-630, DW-599 and FH-574) and to document that the walk down ofinaccessible areas were completed during March/Apnl2013 refueling outage.5. Revised page 124 for Table 6-2 to correct the building and elevation of room CC-1.6. Seismic Walkdown Checklists for 10 components that were identified asinaccessible in the original submittal report.7 . Area Walk-By Checklists associated with the 10 components stated in Item 6 above.8. Revised Area Walk-By Checklist (pages 93-99 of 139) to correct the building andelevation of room CC-l as mentioned in item 5 above.
Findings: No potentially adverse findings were identified for these components and in theassociated rooms. The findings are documented in the attached Seismic Walkdown Checklists (SWC) and the associated Area Walk-By Checklists (AWC).
