NUREG-0737, Item II.P.1.6 Containment Hydrogen Monitors Omaha Public Power District's letter and attached Licensee Event Report (LER 82-017) to the Commission dated October 1, 1982, identifled the valve failures associated with one of the two containment hydrogen monitors. VA-81A was isolated following the valve failures pending completion of repairs.
The valve failures were originally traced to damaged sole-noid coils. Further investigation has shown the cause to be fallure of the Conax feed-through assemblies. The faulty assemblies have been replaced and VA-81A has been returned to service.
