COMANCilE PEAK STEAM ELECTRIC STATION (CPSES)

DOCKET NOS. 50-445 AND 50-446 UNSOUND CONCRETE MORTAR 50AR: CP-87-73 (FINAL REPORT)

Gentlemen:

On August 24, 1987, we verbally notified your Mr. H. S. Phillips of a possible deficiency involving unsound concrete mortar at construction joints.

The conditions addressed in this issue are based upon a site initiated Corrective Action Request (CAR) 87-10 issued as a result of two Nonconformance Reports (NCRs) C-86-200154 and C-85-101580.

Our last interim report on this issue was logged IXX-6884, dated October 28, 1987.

The deficiency described in NCR C-86-200154 is for unconsolidated starter grout at the bottom of a pilaster in a Unit 2 steam generator cubicle.

This NCR was dispositioned to replace the grout.

Calculation No.

15346-CS(S)-145-39B was prepared to verify the structural adecuacy of the pilaster prior to rework.

The results of this calculation incicate that the structural integrity of the construction joint would not be impaired if the joint had remained "as-is."

NCR C-85-101580 described voids, abandoned holes, anchors, and foreign material near the top of a concrete column.

This NCR has been dispositioned to rework the affected area.

Calculation No.16345-CS-(S)-177-398 has been prepared to verify the structural integrity considering the reduced effective area.

The results of this calculation indicate the structural integrity of the column would not have been impaired if the affected area had remained "as-is."

We have concluded that in the event these deficiencies had remaineel uncorrected, no condition adverse to safety would have exist"d.
