CP-87-76 (FINAL REPORT)
Gentlemen:
On August 26, 1987, we verbally notified your Mr. H. S. Phillips of a deficiency involving field drilled cable tray holes.
We have determined that this deficiency is reportable under the provisions of 10CFR50.55(e) and the required information follows.
DESCRIPTION OF PROBLEM Acceptable cable tray / cable tray splice configurations for installation at CPSES are defined by the manufacturer via approved Vendor Drawings or by approved Design Change Authorizations (DCAs).
Unapproved splice plate installations have been observed during field walkdown activities.
Field drilled holes, used to install these plates, have resulted in some unused holes in cable trays and splice plates.
These unused field drilled holes are not considered by CPSES design, and will require extensive engineering evaluation to establish the possible effects on the design adequacy.
This issue is applicable to both Units 1 and 2.
SAFETY IMPLICATIONS Failure of cable trays to perform as intended by design could result in an inability of associated safety-related systems to perform as required for safe operation and shutdown.
We cannot, at this time, provide adequate assurance that this deficiency does not adversely impact the ability of the affected cable trays to perform their intended safety function.
CORRECTIVE ACTIONS We are currently evaluating +.his condition in conjunction with the deficiency logged SDAR CP-86-52, " Cable Tray Splices and Connections," which we deemed reportable under the provisions of 10CFR50.55(e).
