Gentlemen:
By letter logged TXX-6315, dated March 20, 1987, we notified you of a deficiency involving installation of ASME snubber attachment brackets, which we determined to be reportable under the provisions of 10CFR50.55(e).
Our last interim reports were logged TXX-6497, dated June 12, 1987, and TXX-6315 i
dated March 20, 1987. These two interim reports provided specific details of 1
- this deficiency but did not provide a description of all program changes initiated to preclude recurrence of the deficiency.
Program changes not i
previously provided are summarized below.

Our evaluation of project procedures has been completed. As a result of this evaluation, the minimum "c-c" dimensions (centerline-to-centerline distance between the forward and rear mounting brackets pins) for size 3 and size 10 snubber forward brackets have been revised. Also, Project Specification 2323-MS-100, " Field Fabrication and Erection of Piping and Pipe Supports", has 1
been revised to require inspection of size 10 snubbers during installation to l
assure an adequate swing angle cone. The above changes have also been incorporated into pipe support construction procedures CP-CPM-9.10A,
" Installation of Vendor Supplied Component Support Catalog Items" and ACP-11.5, " Component Support Fabrication".
In addition, inspection criteria have been revised in procedures AQP-11.3, " Fabrication / Installation Inspections of Component Supports", AQP-11.5, "ASME Component Installation Verification", QI-QAP-11.1-28, " Fabrication / Installation Inspections of ASME Component Supports, Class' 1, 2, and 3", and CP-QAP-12.1, " Mechanical Component Installation Verification, N-5 Certification".
