DOCKET N05. 50-445 AND 50-446 IMPROPER APPLICATION OF CONSTRUCTION AIDS SDAR: CP-87-59 (FINAL REPORT)

Gentlemen:

On August 20, 1987, we verbally notified your Mr. H. S. Phillips of a deficiency involving vendor supplied cable tray fittings, intended for use solely as construction alignment aids for cable trays, that have been utilized in the systems analyses of cable tray hangers and cable trays as structural members providing tray continuity. We are reporting this issue under the provisions of 10CFR50.55(e) and the required information follows.

DESCRIPTION This deficiency was identified during corrective action evaluations performed for Corrective Action Request (CAR)87-031.

The vendor (T. J. Cope) furnished cable tray fittings (types T-1W and T-11W) to CPSES to aid in alignment of cable tray segments during installation.

Subsequently, some of these fittings were installed as permanent tray fixtures and utilized as structural members providing tray continuity in the systems analysis of cable tray hangers and cable trays. The current Cable Tray Hanger (CTH) Design Validation Program has identified these fittings to be structurally inadequate. In addition, the vendor has advised that these fittings are intended solely for alignment purposes and are not designed to provide structural continuity in the trays. Installation of T-1W and T-11W fittings also creates a potential for rung spacing in adjacent tray segments i in excess of the 9 inch maximum spacing requirement specified in electrical design drawing 2323-El-1701-1 for all ladder type tray fittings and splices.

The root cause of this deficiency is the failure of the design documents associated with installation of the cable tray fittings to conform to the i criteria and bases stated in the FSAR.

'Page 2 of 2 SAFETY IMPLICATIONS The condition represents a significant deficiency in final design as approved and released for construction. Had the condition remained uncorrected, it could have adversely effected the safety of plant operations.

Failure of the cable trays to perform as intended by design could result in an inability of associated safety-related systems to perform as required for safe operation and shutdown.

CORRECTIVE ACTION T-lW and T-11W fittings have been determined to be inadequate. In each instance where these fittings have been employed, Nonconformance Reports (NCR's) will be issued and the fittings replaced with field fabricated fittings through Design Change Authorizations or other approved CPSES program documents. Any deficiencies concerning rung spacing will be documented and corrected during fitting replacement.

The replacement of the cable tray fittings is currently scheduled for completion by March 1, 1988.
