Gentlemen:
On August 24,'1987, we verbally notified your Mr. H. S. Phillips,of a deficiency involving contact surface temperatures that may not have been recorded prior to placement of epoxy type grouts.
This deficiency was l
discovered during third-party investigations and is addressed in Corrective Action Request (CAR)'80X.
This is an interim report of a potentially reportt.ble item under the' provisions of 10CFR50.55(e).
Specifically, manufacturers' specifications for surface temperatures for epoxy grout placement were not incorporated into Specification 2323-SS-09,
" Concrete", and associated procedures, and were subsequently not documented.
i Using' manufacturers' requirements, we are currently reviewing inspection reports and other grout placement documents for surface temperature recordings.
In many cases, temperatures were noted on grout cards when inspections were performed, even though recording this data was not procedurally required.
In cases where surface temperatures were not recorded, the date of the placement and meteorological records can be utilized to l_
establish a placement temperature range.
To date, we have reviewed approximately 700 epcxy grout placements and have identified none that are outside revised specification requirements.
4 Our review of group placements is continuing. Our next report on this issue will be submitted no later than November 13, 1987.
