Dear Mr. Johnson:
On September 41986, we verbally notified your Mr. I. Barnes of a deficiency involving the restraints for the polar crane rail.
Specifically, three rail to rail ground wires have been found broken, two shims have partially dislodged from under the rail, and two cadwelds connecting the ground wire to the rail have been broken. This indicates that movement occurred in the Unit 1 polar crane rail assembly.
Operating experience with the polar crane has shown that the rails tend to creep axially and displace radially during revolution of the crane. The axial creep is cumulative and the rail gaps tend to decrease at some locations and increase at others.
Radial and/or axial rail movements are allowed by the existing rail clips.
Dislodging of shims under the rail, broken ground wires, and broken cadwelds are directly attributed to the radial and/or axial movements of the rails.
The rail movement and associated loss of ground wires and dislodging of shims might have necessitated minor maintenance or repair.
However, they do not affect the aspects of polar crane performance, safety, or reliability that are required for its intended safety function.
Consequently, this issue would not have adversely affected the safety of operation of the plant had it remained uncorrected. This issue is not reportable pursuant to 10CFR50.55(e).
