CP-86-21 (INTERIM REPORT)
Dear Mr. Johnson:
On March 24, 1986, we verbally notified your Mr. T.F. Westerman of a deficiency involving possible discrepancies in vendor (Raychem) methodology for testing of environmental qualification which may render the Raychem Kit NPKS-02-01 unacceptable for specific published applications. This is an interim report of a potentially reportable item under the provision of 10CFR50.55(e). We have submitted interim reports logged TXX-4772, TXX-4830, and TXX-4913, dated April 23, 1986, May 28, 1986, and July 16, 1986, respectively.
i The specific deficiency involves a heat shrink kit used for in-line butt-splices. The kit consists of two inner sleeves which are enclosed by an outer sleeve. When overlapped at the butt-splice, the effective outside diameter of the inner sleeves is greater than the specified useful range of the outer sleeve. Raychem has indicated the condition is the result of an earlier design deficiency in which the American Wire Gauge (AWG) standard was used to size the outer sleeves as opposed to specifying a kit-specific outside diameter.
Discussions with the supplier indicate the Raychem design deficiency has been corrected and CPSES applications of the kit have been specifically qualified.
Receipt and review of reporting documentation is scheduled for completion on September 5, 1986.
We are continuing our evaluation and anticipate submitting our next report by September 26, 1986.
