COMANCHE PEAK STEAM ELECTRIC STATION DOCKET NOS. 50-445 AND 50-446 6.9KV SWITCHGEAR BREAKER JACK SLEEVE WELD FAILURE SDAR:
Dear Mr. Johnson:
On March 11, 1986, we verbally notified your Mr. T. F. Westerman of a deficiency involving a weld failure in a component of a Class IE 6.9KV switchgear breaker.
This is an interim report of a potentially reportable item under the provisions of 10CFR 50.55(e).
Inspection activities are underway for all critical breakers.
Non-critical breakers are being checked using a statistical random sample.
These activities should determine if the failure is isolated or exists as a result of a common failure mode.
We are continuing our evaluation and anticipate completion of our final report by May 15, 1986.
