Dear Mr. Hunter:
In accordance with 10CFR50.55e, we have concluded that the deficiencies discovered during reinspection efforts of T.R.T Issues 1.a.2 and 1.a.3 constitute a reportable condition.
To ensure that the equipment involved will satisfactorily perform its intended safety functions we are conducting a significant reinspection of the work performed by the original inspectors.
In addition, one inspector has been terminated partly because of falsification of inspection records.
