r TJX-4001 July 7, 1983 ATTACHMENT LETDOWN HEAT EXCHANGER ANCHORS Description of Deficiency In reviewing the mounting installation of the letdown heat exchanger for an unrelated issue, site engineering observed the anchor bolt configuration.
The mounting design calls for slotted holes on one end of the heat exchanger to allow thermal expansion. The anchor bolts were found to be bound in the end of the slotted holes prohibiting movement.
The letdown heat exchanger is not required to shut down the plant or mitigate the consequences of an accident. A preliminary evaluation by the supplier (Westinghouse) has indicated the equipment would probably not be overstressed under normal conditions but, under OBE and SSE conditions, the potential exists for the components to be overstressed.
Safety Implications In the event the existing conditions had gone undetected, the structural and pressure boundary integrity of the letdown heat exchanger and associated components could not be assured during a seismic event.
Cor_rective Actions The mounting configuration will be reworked in accordance witi..he supplier's recommendations. The revised configuration will conform to original mounting requirements. The rework will be completed no later than September 1,1983.
Based upon our examination of other equipment, we have determined the concern is not generic.
