The inspectors identified a noncited Sever … The inspectors identified a noncited Severity Level IV violation of 10 CFR 50.9 requirements when a security officer deliberately falsified fire watch logs. Specifically, on September 1, 2008, the officer failed to perform two fire watch tours and then signed the fire watch logs indicating that the tours were completed as required. This issue was entered into the licensees corrective action program as Palo Verde Action Request 3219290.The failure to provide complete and accurate information on the fire watch log is a performance deficiency. This issue was dispositioned using traditional enforcement due to the willful aspects of the performance deficiency. Furthermore, the failure to provide complete and accurate information has the potential to impact the NRCs ability to perform its regulatory function. In accordance with Section IV.A.4 of the Enforcement Policy, this issue is considered more than minor due to the willful aspects of the performance deficiency. In accordance with the guidance in Supplement I of the Enforcement Policy, this issue is considered a Severity Level IV noncited violation because it was identified by the licensee, involved isolated acts of a low-level individual, and was addressed by appropriate remedial action.
