No findings of significance were identifie … No findings of significance were identified with the conduct of forced outage activities. The inspectors identified an Unresolved Item concerning events that occurred on January 17, 2010, when a radiation protection technician (RPT) observed a radiation worker with an arm inside a highly radioactive contaminated system without the proper equipment to control the spread of radioactive contamination. The radiation worker remained inside the drywell and performed additional work, after the RPT made this observation. Subsequently, the radiation worker was allowed to leave the Radiologically Controlled Area (RCA) without successfully passing a contamination monitor and without additional radiological controls. Radioactive contamination was later identified when the radiation worker caused contamination monitor alarms. At the time of the inspection, the radiation workers access to the RCA was restricted and the licensee was still in the process of compiling all of the facts surrounding the event(s). Additionally, an evaluation of the radiological hazards of the work performed was still being developed. As a result, any radiological consequence for the worker was unknown. Similarly, the inspectors could not evaluate the consequence of any apparent improper radiological controls.
