Technical Specification 5.4.1.a, Procedure … Technical Specification 5.4.1.a, Procedures, requires, in part, that written procedures be established, implemented, and maintained as recommended in Regulatory Guide 1.33, Revision 2, Appendix A, dated February 1978. Paragraph 9.a requires that maintenance that can affect the performance of safety-related equipment should be properly preplanned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances. Contrary to the above, the licensee failed to implement written procedures as recommended in Regulatory Guide 1.33, Revision 2, Appendix A, dated February 1978. Specifically, on May 13, 2002, August 9, 2003, December 2, 2005, September 28, 2007, September 21, 2009, and April 18, 2011, the licensee failed to inspect the air-side of the Division 1 critical switchgear room air handling unit WMA-AH-53A in accordance with written instructions because the air handling unit roll-filter was not removed to allow for a comprehensive inspection of the cooling coil surface. This finding was identified by the licensee and entered in the licensees corrective action program as Action Requests AR 285811. This finding was determined to be of very low safety significance because the failure to perform a required inspection did not result in a loss of functionality of service-water-supplied heat exchangers.of service-water-supplied heat exchangers. +
