A non-cited violation of Technical Specifi … A non-cited violation of Technical Specification 5.4.1.a was identified for the failure to follow radiation work permit requirements to brief workers on the radiological conditions in the work area and to provide workers current radiological survey information. On August 28, 2012, during the biennial graded emergency preparedness exercise, mock repair teams entered the radiologically controlled area without being briefed on the actual radiological conditions and without being provided with current radiological survey information. The licensee entered this issue into their corrective action program as Action Request AR 269791. The finding was more than minor because it was associated with the Occupational Radiation Safety Cornerstone exposure control attribute of program and process and it affected the cornerstone objective to ensure the adequate protection of the worker health and safety from exposure to radiation from radioactive material because it could have increased worker exposure while in the radiologically-controlled area. The inspectors evaluated the finding using Inspection Manual Chapter 0609 Appendix C, Occupational Radiation Safety Significance Determination Process, dated August 19, 2008. The inspectors determined that the finding was of very low safety significance because it did not: (1) involve ALARA planning and work controls; (2) result in an overexposure; (3) involve a substantial potential for overexposure; and (4) compromise the licensees ability to assess dose. In addition, the finding had human performance cross-cutting aspects associated with resources because the licensee did not ensure that complete, accurate, and up-to-date documentation (radiological surveys) were adequate to ensure radiological safety.re adequate to ensure radiological safety. +
