TS 3.9.3 requires the licensee to immediat … TS 3.9.3 requires the licensee to immediately suspend core alterations and movement of fuel assemblies within containment when one or more containment penetrations are not in the required status. Contrary to the above, the licensee performed maintenance on a Residual Heat Removal (RHR) pump suction relief valve during core alterations which resulted in containment refueling integrity being lost for approximately 10 minutes. On October 29, 2010, the licensee discovered one of the containment penetrations had direct access to the outside atmosphere during core alterations. On October 28, 2010, the licensee had commenced core alterations in the morning hours with containment refueling integrity established. The licensee undertook a scheduled maintenance activity on October 29, 2010, removing the RHR pump suction relief on train B RHR. The removal of the relief valve from the system created an opening inside containment providing direct access from the containment atmosphere to the auxiliary building during the time the relief valve was not installed. This condition lasted for approximately 10 minutes. The licensee installed the replacement relief valve at the point of discovery re-establishing containment refueling integrity. The licensee entered this condition in its CAP as CR 2010101103. This finding was assessed using IMC 0609, Phase 1 screening worksheets of Attachments 4 and Appendix H relating to Containment Barrier Integrity, and determined to be of very low safety significance (Green), because the finding represents a shutdown condition in Mode 6 where decay heat is relatively low and occurred greater than 8 days after the start of the refueling outage.s after the start of the refueling outage. +
