Violation of TS 5.4.1, \'Procedures\' for Violation of TS 5.4.1, \'Procedures\' for not properly conducting and documenting maintenance on the Backup Indicating Panel, a remote panel required for safe shutdown of Unit 1. Two thermocouple input wires were discovered disconnected internal to the panel on October 31 during a refueling surveillance test from a previous maintenance activity. The licensee documented this issue in CR 10-85168.
