A licensee-identified violation was discovA licensee-identified violation was discovered on March 13, 2009 for Unit 2 of TS 3.6.1.3 which states, each primary containment isolation valve (PCIV), except reactor building-to-suppression chamber vacuum breakers, shall be operable during Modes 1, 2 and 3, when associated instrumentation is required to be operable per LCO 3.3.6.1, \"Primary Containment Isolation Instrumentation.\" Surveillance SR 3.6.1.3.10 required combined leakage rate for all secondary containment bypass leakage paths be less than 0.009 La (allowable leakage). Contrary to this requirement, on March 13, 2009, it was discovered during the Unit 2 refueling outage (RFO), that A feedwater line penetration containing secondary containment bypass valves 2B21- F010A and 2B21-F077A would exceed the TS allowable leakage rate for that leakage path. The significance of this finding was screened using the Phase 1 of the SDP in accordance with NRC IMC 0609 Attachment 4, Table 4a. The analysis determined the significance of the finding to be Green, because the finding did not represent an actual open pathway in the physical integrity of reactor containment. Corrective actions taken by the licensee included repairs to the 1B21-F010A and 1B21-F077A valves followed by satisfactory post-maintenance testing to verify leakage was within limits prior to plant restart.
