The licensee identified an NCV of TS 5.4.1 … The licensee identified an NCV of TS 5.4.1, procedures, for failing to establish a procedure for testing of containment equalization fans. Specifically, contrary to the requirements of TS 5.4.1 to establish procedures identified in regulatory guide 1.33, the licensee did not establish a procedure for surveillance testing of a section of the circuity for the containment equalization fan. On May 15, 2016, while performing a walkdown for a planned project on the solid state protection system, the licensee identified that a portion of the circuit was not verified to ensure the integrity of the circuit path to actuate a containment isolation fan. After discovery, the licensee performed a test to verify circuit integrity for containment equalization fans on both units. The condition of an inadequate surveillance existed for many years. Since all fans passed the surveillance test, the licensee confirmed the failure to test had not masked an inoperable condition. The issue was more than minor because it impacted the mitigating system cornerstone objective of ensuring the reliability of systems that respond to plan events. Because the licensee demonstrated that the circuit was operable, the inspectors concluded it was not inoperable longer than permitted by technical specifications and determined that issue was of very low safety significance.
