Appendix B, Criterion XVI in 10 CFR Part 5 … Appendix B, Criterion XVI in 10 CFR Part 50 requires, in part, that measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected. Contrary to this, on October 15, 2010, the licensee determined that they had failed to promptly identify deficiencies associated with the Unit 2 safety-related containment divider barrier seal bolting. The licensees evaluation concluded that TS 3.6.13, Divider Barrier Integrity, had not been met for a period beyond the allowed outage time due to the failure to identify the deficiencies. The licensee also concluded that the deficiencies were likely present during previous inspection opportunities but were not identified and/or corrected. In addition, the licensee identified that a similar failure to promptly identify and correct divider barrier deficiencies had been identified in 2006, and corrective actions put in place at that time to enhance the divider barrier surveillance were not implemented. For corrective actions, the divider barrier seal bolting deficiencies were repaired during the outage. Also, this issue was entered into the licensees CAP as AR 2010-10838 and AR 2010-10939 to reevaluate the divider barrier surveillance process and to analyze the cause for the repeated issues. The finding is of very low safety significance because it screened as Green per IMC 0609 Attachment 4 by answering no to all questions in Table 4a under the Barrier Cornerstone.in Table 4a under the Barrier Cornerstone. +
