Prairie Island Technical Specification 3.0 … Prairie Island Technical Specification 3.0.6 requires, in part, that an evaluation shall be performed in accordance with Technical Specification 5.5.13, Safety Function Determination Program, when a supported system LCO is not met solely due to a support system LCO not being met. Specifically, if a loss of safety function is determined to exist by the Safety Function Determination Program, the appropriate Conditions and Required Actions of the LCO in which the loss of safety function exists are required to be entered.Contrary to this TS requirement, between August 18 and 22, 2017, control room operators did not evaluate Unit 2 A Component Cooling, Auxiliary Feedwater, and Cooling Water supported system LCOs while the 121 Safeguards Chilled Water support system LCO was not met. As a result, the appropriate Conditions and Required Actions were not entered during Unit 2 B Component Cooling and Auxiliary Feedwater supported system maintenance and testing activities for which a loss of safety function existed. Because the inspectors answered No to all questions under Exhibit 2.A of IMC 0609, Appendix A, The Significance Determination Process for Findings at-Power, the finding screened as very low safety significance (Green). Specifically, the finding did not represent (result in) an actual loss of function of two separate safety systems out-of-service for greater than their TS-allowed outage times. The above issues were documented in the licensees CAP as CAP 501000001929.
