Technical Specification 5.7.2, states, in Technical Specification 5.7.2, states, in part, that each high-radiation area, accessible to personnel with radiation levels > 1000 mrem/hr at 30 cm (12 in.) from the radiation source or from any surface which the radiation penetrates shall have doors that are locked to prevent unauthorized entry. Contrary to the above, on April 26, 2016, the licensee identified the locking mechanism for a door was non-functional and could not prevent unauthorized entry to the area. Specifically, a worker intentionally challenged the locking mechanism to the Unit 2 low pressure heater bay door when the latch opened. The individual left the area and later reported the issue to the radiation protection staff that promptly secured the area with an alternate locking mechanism and determined that the dose rates exceeded 1000 mrem/hr inside the area. The licensee documented this issue in Issue Report 2661096 and reported a PI occurrence under the Occupational Radiation Safety Cornerstone. The inspectors determined that this issue was of very low safety-significance (Green) after reviewing IMC 0609, Appendix C, Occupational Radiation Safety Significance Determination Process, dated August 19, 2008. The inspectors determined that it was not an as-low-as-reasonably-achievable planning issue, there was neither overexposure nor substantial potential for an overexposure, and the licensees ability to assess dose was not compromised.
