Title 10 CFR 50, Appendix B, Criterion XVI … Title 10 CFR 50, Appendix B, Criterion XVI, Corrective Action, requires, in part, that measures shall be established to assure that for significant conditions adverse to quality, the cause of the condition is determined and corrective action(s) taken to preclude repetition. Contrary to this requirement, the licensee did not take adequate corrective actions to prevent recurrence for a significant condition adverse to quality regarding the failure of a fire door fusible link on July 26, 2007. The licensee entered the issue into their corrective action program as CAP 01194446. Based upon consultation with a Region III SRA who determined that the delta CDF for this finding was much less than 1E-6, it was considered to be of very low safety significance (Green)be of very low safety significance (Green)  +
