Specifically, inspectors determined that TVA did not adequately implement 0-TI-362, Inservice Testing of Pumps and Valves. , which required that a Service Request (SR) be documented in the CAP when pumps are found to be in the Alert Range during inservice testing. The inspectors determined that the failure to implement procedure 0-TI-362 constituted a performance deficiency. This performance deficiency was determined to be more than minor in accordance with IMC 0612 Appendix B, Issue Screening because if left uncorrected, the failure to enter degraded conditions in the CAP has the potential to lead to a more serious safety concern. The inspectors screened this finding in accordance with Inspection Manual Chapter (IMC) 0609, Significance Determination Process, Attachment 4, Phase 1 - Initial Screening and Characterization of Findings, and determined the finding was of very low safety significance (Green). The cause of this finding was directly related to the crosscutting area of problem identification and resolution, the component of the corrective action program and the aspect of issue identification; because the licensee failed to implement the corrective action program.o implement the corrective action program. +
