A self-revealed finding of very low-safety … A self-revealed finding of very low-safety-significance and associated NCV of TS 5.7.1, occurred when an individual entered a high radiation area without proper authorization. The individual was not knowledgeable of dose rates in the area. The licensee has entered the condition into their CAP. Corrective actions included counseling of the worker and the error was discussed with all Nuclear Plant Operators at shift turnover. The finding was more than minor because it is addressed in Example 6.h of IMC 0612 Appendix E, Examples of Minor Issues. Additionally, the inspectors determined that the finding was more than minor because it is associated with the program and process attribute, and affected the Occupational Radiation Safety Cornerstone objective to ensure the adequate protection of the worker health and safety from exposure to radiation from radioactive material during routine civilian nuclear reactor operations. This finding was assessed using IMC 0609, Attachment C for the Occupational Radiation Safety SDP and determined to be of very low safety significance (Green) because this failure did not involve ALARA Planning or Work controls; did not result in an overexposure or substantial potential for overexposure and there was not a compromised ability to assess dose. The finding was caused by the worker that did not ask for a peer check before entering the posted high radiation area. Consequently, this finding had a cross-cutting aspect in the area of human performance work practices.
