However, there was an event that occurred … However, there was an event that occurred during the refueling outage where an individuals respiratory protection equipment failed during the removal of the reactor head o-ring. This event resulted in an intake of radioactive material. However, the internal dose assessment was not complete because the event occurred within an area where it was difficult to detect which radionuclides were present. In accordance with plant procedures, samples (i.e., area contamination and in vitro bioassay) were collected and sent to a contracted off-site facility to perform analysis for those difficult to detect radionuclides. The results from this analysis were not available during the inspection. Additionally, the cause(s) for the respiratory equipment failure was still under evaluation during the inspection. Therefore, this issue remains under review by the NRC and is categorized as an Unresolved IteRC and is categorized as an Unresolved Ite +
