Title 10 CFR Part 50, Appendix B, CriterioTitle 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, requires, in part, that activities affecting quality be prescribed by documented procedures of a type appropriate to the circumstances and be accomplished in accordance with these procedures. The licensee established procedure QCOP 650007, Racking in a 4160 V Horizontal Type AMHG or G25 Circuit Breaker, as the implementing procedure for installing fuse blocks in safety-related breakers, an activity affecting quality. Contrary to the above, prior to August 21, 2015, the licensee failed to have a procedure for installing fuse blocks in safety-related breakers. Specifically, QCOP 350007 failed to ensure that fuse blocks for safety-related 4160 Volt breakers were properly installed to ensure the breakers would perform their function. The procedure did not provide the operators guidance to ensure the fuse blocks were fully seated, and on August 21, 2015, following post-maintenance testing for the Unit 1A RHR pump breaker and the system being declared operable, an equipment operator on rounds identified that the breaker closing springs were not charged. The licensee determined that the breaker fuse blocks were not fully seated following breaker maintenance. The licensee captured this issue in the CAP as IR 2550801. The inspectors evaluated the finding under Inspection Manual Chapter 0609, Appendix A, The SDP (Significance Determination Process) for Findings At-Power, issued June 19, 2012. The inspectors answered No to questions A1A4 in Exhibit 2, Mitigating Systems Screening Questions, and determined the finding was of very low safety significance (Green).s of very low safety significance (Green).  +
