SUBJECT:

UNPLANNED AIRBORNE RELEASE OF RADI0 ACTIVE N0BLE GAS On April 18, 1988 with the reactor in Mode 5 and the containment hatches open, workers removed a flange from the pressurizer relief tank releasing radioactive noble gas. The in-containment airborne activity and effluent radiation monitors showed increasing levels while the licensee secured the ventilation purge and evacuated workers from the containment.

Shortly thereafter, the level of airborne activity resulted in a containment purge isolation signal (ESF actuation). The licensee reports that about 4.4 curies of xenon-133 were released. This corresponds to approximately one percent of the Technical Specification limit. Seven workers were cor.taminated with xenon-133 and the maximum whole body exposure was measured by self reading dosimeter to be 14 millirem. The release was caused by inadequate procedural control / work instructions relative to removing the flange without first purging the tank.

The State of Oregon, Department of Energy, was notified.

The licensee issued a press release.
