The information is as initially received without verification or evaluation, and is basically all that is known by Region IV staff in Arlington, Texas on this date.

DAMAGED MOISTURE DENSITY GAUGE AT A CONSTRUCTION SITE (STATE OF COLORADO LICENSEE)

On October 27,1998, Kleinfelder, Inc., a Colorado State licensee contacted the National Response Center and the State of Colorado to inform them of an event at a temporary job site in Walsenburg, Colorado. It was reported that the accident occurred when a pavement roller ran over a Troxler moisture density gauge (Model 3430) on October 27,1998. The gauge contained an 8 millicurie cesium-137 sealed source and a 40 millicurie americium-241/berylium sealed source.

The moisture density gauge's outside plastic case was broken, and the gauge electronics were damaged. It was recorted that the sealed sources were not damaged. The gauge was surveyed and no abnormal radiation levels were found. The gauge will be transported back to Kleinfelder's home office in Pueblo, Colorado, and then shipped back to Troxler for repairs.

The State of Colorado was informed of the event by the licensee. The State has scheduled an inspection at the licensee's office on Wednesday, October 28,1998.

Region IV received notification of this occurrence by the NRC Operations Center at 4:00 p.m. (CDT) on October 27,1998. Region IV has informed OSP and NMSS. l This information has been discussed with the State of Colorado and is current as of 4:30 p.m. (CDT) October 27,1998
