DAMAGED TROXLER MOISTURE DENSITY GAUGE (SOURCE NOT DAMAGED) -
On July 21,1998 the state of Washington notified the NRC of a damaged moisture / density gauge. Based on inforrnation provided by the State, on July 21,1998, a Troxler Moisture Density Gage Model #3430, serial #28458, owned by Earth Consultants, Inc., Bellevue, WA, was hit by a bulldozer at a temporary job site near Federal Way, WA. At 8:30 a.m. PDT on July 16,1998, the bulldozer hit the gauge which was not in use at the time. The blade skimmed over the gauge bending the hanole and the source rod which was in the safe /up positior. The electronics were also crushed. The licensee representative closed the open I
shutter 'the gauge, moved the gauge to the licensee's office, isolated the immediate area, and nott.1 the licensee Radiation Safety Officer, and the Washington State Division of Radiation Protection. On July 17,1998, a representative from the Washington State Division of Radiation Protection, examined the gauge and determined by wipe test that no leakage had occurred and that the gauge could be safely retumed to the manufacturer for repair.
The gauge contains 8 millicuries of Cs-137 and 40 millicuries of Am-241/Be.
Region IV was notified of the event by a Washington representative at approximately 9:36
