LOSS OF PROCESS CONTROL LEVEL GAUGE CONTAINING CESIUM 137 A representative of the State of Texas notified the NRC Operations Center et 9:39 a.m. (CST) on l

February 24,1998, that La Gloria Oil and Gas, a Texas licensee, had informed them of the loss of a process level gauge containing 500 millicuries of Cesium-137, The level gauge, manufactured by TN Technologies, Model 5191, had been used for process control of their coke operations and was one of three gauges located inside a process tank. The Texas licentiee stated that the source which was contained in a tungsten tube, may have fallen off into the tank cs early as December 6 1997, it was determined that two facilities (Great Lakes Carbon. Port Arthur, Texas, and Reynolds Aluminum, Baton Rouge, Louisiana) had received shipments of ccie product since that date.

The state of Texas dispatched an inspector to La Gloria Oil and Gas on February 23,1998.The inspector did not find any contamination at the facility. In addition, on February 24,1998, Texas l

dispatched an inspector to the Great Lakes Carbon facility in Port Arthur, Texas.

The state of Louisiana is celebrating a Mardi Gras holiday and notification was made by the state of Texas to the Louisiana Protection Division Duty Officer on February 25,1998. Most businesses including Reynolds Aluminum, are closed because of the holiday.

RIV received notification of this occurrence by the NRC Operations Center by telephone at 10 a.m (CST) on February 24,1998.

Region IV has informed OSP.

This information has been oiscussed with the state of Texas and is current as of 4 p.m. (CST),

February 24,1998.
