LOST AND RECOVERED RADIOGRAPHIC EXPOSURE DEVICE On the morning of November 5,1997, the Louisiana Department of Environmental Quality (DEQ) reported tu the Office of State Programs that a SPEC Model 150 eadiographic exposure device containing a 102 curie iridium-192 sec;ed source fell from the back of a licensee truck on a state highway between Emoire and Belle Chasse, Louisiana. The licensee noticed that the device was missing after arriving at its facility in Belle Chasse.
A DEO employee, equipped with a radiation detection survey meter, was dispatched to search for the device along a road where the device is believed to have f allen from the truck.
-On the af ternoon of November 5th, the licensee was notified by a representative from the Louisiana Department of Transportation (LDOT) that the device had been recovered. LDOT had been traveling on the road where the device fail from the truck earlier in the day, and observed the exposure device laying on the road. The LDOT workers were cognizant of what the device was, and promptly returned the device to the licensee.
The device was found _with the source secured in the shielded position and appeared unNmaged.
NRC's Office of State Programs received notification of this occurrence i by telephone from the DEQ on the morning of November 5,1997.
Region IV has informed I; MSS, State Programs, and AEOD.
This information has been discussed with the DEO and is current as of 3:30 p.m. (CST) on November 5,1997.
