THEFT OF RADIOGRAPHIC EXPOSURE DEVICE <
At approximately 9:30 a.m. (CDT), the radiation safety officer (RSO) for Bill Miller, Inc. (BMI) contacted Region IV to report the thef t of a radiographic exposure device in Tulsa, Oklahoma.
The RSO subsequently notified the NRC Operations Center. Information provided by the RSO is summarized below.
On the evening of September 17,1997, a licensee radiographer parked a BMI vehicle at a motelin Tulsa, Oklahoma, where he was staying for the night. The vehicle had a radiographic exposure device stored in the darkroom of the vehicle. The device is an INC Model IR 100 (Serial No. 4194) and contained a 16 curie iridium 192 sealed source (Serial No. 6782). The device bears labels indicating the isotope, serial number, nominal activity, and radiation label.
The source was secured and locked in the shielded position, and the device was blocked and braced in a shipping overpack container. However, the camera lock box key was in the vehicle.
The radiographer noted that the vehicle was missing the following morning, September 18, 1997.
The licensee has notified the Tulsa Sheriff's Department and is drafting a press release to request assistanco wm the public with recovery of the device.
The state of Oklahoma has been informed.
Region IV received notification of this occurrence by telephone from the licensee at 9:30 a.m.
(CDT) on September 18,1997. Region IV has informed NMSS, This information has been discussed with the licensee and is current as of 10 a.m., September 18,1997.
