RADIOGRAPHY SOURCE DISCONNECT EVENT The Louisiana Department of Health notified the Region IV Office of an event involving a Louisiana licensee which occurred while the licensee war working under reciprocity in Tennessee. The event occurred on May 22,1997, but was not reported to Louisiana until May 30.

Upon completion of a radiography exposure at a temporary job site in Counce, Tennessee, the  ;

drive cable disconnected from the source assembly. Further review revealed that the drive l cable connector detached from the drive cable leaving the source assembly in the guide tube.

The two radiographers removed the collimator from the guide tube, inserted another drive cable at that point, and pushed the source assembly into a shielded position in the exposure device. Radiography then resumed using a nondefective drive cable.

Each of the two radiographers wore 5,000-mR pocket dosimeters which registered 480 mR l and 500 mR, respectively, after the recovery effort. Dosimeter badges have not yet been sent to the processor. The sealed source used was a 70-curie iridium-192 source installed in a SPEC 150 exposure device.

The defective drive cable was manuf actured by an af termarket supplier, industrial Radiography Maintenance and Supply, Morgan City, Louisiana. Louisiana has notified Tennessee and plans further review of the event and the associated equipment.

Region IV notified Region 11 and NMSS. This information is current as of May 30,1997, at 4:30 pm CDT.
