The information is as initially received without verification or evaluation, and is basically all that is known by Region IV staff in Arlington, Texas on this date.

THEFT OF RADIOGRAPHIC EXPOSURE DEVICE (EVENT NUMBER: 32084) l At approximately 9:00 a m. (MST) the assistant radiation safety officer for Mattingly Testing l Services, Inc. (MTSI) contacted the NRC Operations Center to report the theft of a radiographic exposure device from M TSI's f acility in Billings, Montana. The assistant radiation safety officer subsequently contacted the Region IV office at approxinately 11:50 a.m. (MST) to discuss the incident with Region IV staff. Information provided by the MTSI representative is summarized below.

At approximately 7:30 a.m. (MST) on April 4,1997, the assistant radiation safety officer reported to MTSI's Billings office and noted the door to a storage shed (an enclosure separate from the office budding) was open. The shed is used to store radiographic equipment when not in use, including exposure devices. The assistant examined the door and inside the shed and noted that it did not appear that entry into the shed was forced, although the door is routinely secured by a keyed lock. The assistant noted that an electrical generator and an exposure device were missing from the shed. MSTI had only one exposure device stored in th6 shed because it housed a source that was of lower activity and was not frequently used by the staff. The exposure device had been chained, and locked, to the generator. The assistant stated that he last icoked at the shed door to verify that it was secured on Monday, March 31.

The exposure device is an INC Model IR-100 (Serial No. 4136) and housed a 25 curie iridium-192 sealed source (INC Serial No. 6420). The device bears labels indicating the source strength, serial number and nominal activity; the device serial number and radiation label; and the licensee's name and emergency phone number. The device was locked and the key to the lock was stilllocated at the licensee's facility. No ancillary equipment (i.e., drive control The licensee has notified the Yellowstone County Sheriff Department, whth will also ensure notification of the Billings Police Department. The licensee plans to issue a press release and offer a reward for return of the device.
