SYNCOR RADIOPHARMACY CONTAMINATED SHIPPING CONTAINER On October 8,1996, Mercy Hospital, in the St. Paul, Minnesota area, received a radiopharmaceutical shipment from the Syncor international Corporation. The shipping package, an ammo box containing technetium-99m, was surveyed and wiped for contamination by the hospital nuclear medicine staff and no unusual readings were initially noted. However, the nuclear medicine staff noted a small, discolored area (brown) on the exterior of the package and performed a more detailed survey of this area. A survey reading of 25 mr/hr was obtained at the discolored area of the package surface.
Removable contamination samples were taken directly from the area of discoloration.
Analysis of the wipe samples by the hospital revealed a contamination reading of approximately 180,000 to 700,000 counts per minute (dry and alcohol wipes respectively) from the spot. No contamination was found on the inner packaging. Syncor personnel later identified the radioactive contaminant as technetium 99m based on gamma spectroscopy analysis. Follow-up surveys of the Syncor delivery vehicle, driver, St. Paul radiopharmacy, and Mercy Hospital revealed no contamination.
Syncor's corporate radiation safety staff has confirmed that the incident is being investigated by pharmacy managers at the St. Paul facility. The pharmacy radiation safety officer planned to conduct train'ng with all personnel responsible for packaging radioactive materials and is examining the need for additional actions. Region IV personnel will review the licensee's corrective actions during a future inspection.
The state of Minnesota will be informed by Region Ill. NMSS has been informed.
Region IV received notification of this occurrence by telephone from the Syncor Corporate Radiation Safety Officer at 2:30 PM (PST), October 8,1996. Region lit received notification of the occurrence from Mercy Hospital.
