SUBJECT:

STOLEN GAUGE DESCRIPTION: On October 5, 1993, the licensee's Radiation Safety Officer (RS0) telephoned Region IV to report the theft of a Humboldt moisture / density gauge. Upon completing work using the subject gauge in Lenexa, Kansas, on October 4,1993, a i

licensee employee did not return the gauge to the licensee's facility in Kansas City, Kansas, but took the gauge to his home in Independence, Missouri. The l

employee left the gauge chained to the bed of the truck, which he parked in front of his home at about 6:20 p.m.

At about 7:30 the next morning, October 5, 1993, the l

employee found the chain cut and the gauge missing.

The employee notified the RSO, who then notified the Independence, Missouri, Police Department.

The stolen gauge, which contained 10 millicuries of Cs-137 and 40 millicuries of Am-241/Be, was enclosed in its unlocked transportation container. The RSO stated that the radiation levels at the surface of the container are very low.

The state of Kansas, NMSS, and NRC Region III have been informed.

Region IV received notification of this occurrence by telephone from the licensee's RSO at 9:45 a.m. on October 5, 1993.
