SUBJECT:

SHIPNENT RECEIVED WITH CONTAMINATED SHIPPING TAPE The Texas Bureau of Radiation Control received a written report from their licensee and notified Region IV on September 13, 1989, of a shipment received with contaminated shipping tape. The licensee, through package opening procedures, identified one small point of contamination, which proved to be an iridium-192 seed in a teflon sleeve. The seed was stuck to the adhesive side of the shipping tape.

The shipment was received from Best Industries, Springfield, Virginia, on August 4, 1989. The hospital had notified the shipper immediately. The RSO at the hospital estimated the seed at approximately i mci iridium-192. Texas inspectors are investigating the incident and will examine the seed, which is still in possession of the hospital's RSO.

NRC offices, NMSS, Region II, and SLITP, have been notified.

There is no apparent media interest and no press releases are planned.
