OPERATOR EXAMINATION FAILURES As part of its )lan to improve operator performance, the licensee developed a hig1 intensity training 3rogram for licensed and non-licensed operators. The program commenced on iarch 10. 1997. During the week of March 17. 1997, through a niid-term performance evaluation of the first operating crew to attend the training, the licensee identified operator

performance deficiencies and decided to accelerate testing of the remaining operating crews. The licensee has 6 operating crews totalling 60 NRC-licensed operators.

As of March 27, 1997, the licensee had completed evaluations of three operating crews (divided into six simulator cren). Five of the six simulator crews were rated as unsatisfactory by the licensee. Of the 31 operators evaluated thus far. 6 passed. 18 faiied as individuals, and 7 failed because their crew failed but did not fail mdividually.

1 The operators that failed were suspended from licensed duties pending remediation training. The licensee is providing either short term (one day to one week) or long-term remediation based on each operator's performance. Evaluation of the remaining three crews will be conducted after the crews 1

have received training on new operating standards and is expected to be completed by April 15, 1997.

Eight non-licensed operators had also been evaluated as of March 27, 1997. Of these eight operators, two passed, two were scheduled fo.r short-term remediation, and four were removed from shift duties until they undergo long-term remeaiation and re-evaluation.

An NRC Region III (Chicago) operator licensing examiner observed the evaluation of one of the crews and the NRC plans a review of the licensee's remediation training during the week of April 7,1997. The NRC plans to complete by A)ril 30, 1997, a broader review of the examination results to determine t1e nature and actual significance of the failures.

There has been extensive news media interest in the licensee's operator examination failures.

The State of Illinois has been informed. The information in this preliminary notificatica has been reviewed with licensee management. This information is current as of 11:00 a.m. on April 3,1997.
