RADI0 PHARMACEUTICAL PACKAGE EXCEEDS CONTAMINATION LIMITS On February 25, 1997, the licensee's consultant notified NRC Region III '

(Chicago) that a package (ammo box) with radioactive contamination exceeding Department of Transportation limits was received earlier that day at the licensee's Livonia Internal Medicine Facility Livonia.

Michigan. The package was shipped by the Medi-Physics nuclear pharmacy.

Upon receipt of the package, the licensee's technician performed an initial wipe test on the outside of the package which indicated 27.000 disintegrations per minute (dpm). (The DOT limit is 2200 dpm/100 sq cm).  ;

Additional wipe tests indicated 6.270 dpm on the right side of the package, 4,681 dpm on top. 3,059 on the left side, 1,573 dpm on back. l 1.249 dpm on bottom, and 3.546 on front of the package. A survey of the I package indicated exposure rates between 1 to 1.5 millirem per hour. i The package contained 3 unit doses, each with 25 millicuries (925 MBq)  ;

of technetium-99m. Internal wipes of the package and contents were negative. The ammo box was placed in storage.  ;

i The licensee notified Medi-Physics who performed surveys of the delivery vehicle and driver. Survey results were negative. Two NRC inspectors were performing an inspection of the licensee during the time the .

package was delivered. The inspectors will review the event during an inspection at Medi-Physics on February 26, 1997.
