PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PNO-III-97-002 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by Region III staff (Lisle Illinois) on this date.

PACKAGE EXCEEDS DOT REQUIREMENTS On January 9,1997, the licensee notified NRC Region III (Chicago) that a 3ackage was received from Syncor International's nuclear pharmacy. Grand Rapids, Michigan, on January 8,1997, which exceeded the Department of Transportation (DOT) regulatory limits. Upon receipt of the package (on January 8,1997). the-licensee's technician performed a wipe test and found 11.000 dpm on the lip of the container (DOT's limit is 6600 dpm).

The package contained 40 millicuries (1,480 MBq) of Technetium-99m.

The licensee's Radiation Safety Officer and Syncor were notified by the technician. Syncor performed a status check of the other shipments in the vehicle and informed the licensee that no additional contamination issues were identified. The licensee decontaminated the container and used it for a return shipment to Syncor International, Grand Rapids on January 9, 1997.

The State of Michigan, the NRC Office of Nuclear Material Safety and Safeguards and NRC Region IV have been notified. The information in this preliminary notification has been reviewed with licensee management.

NRC Region III was notified of the event at 10:45 a.m. (CST) on January 9.1997. This information is current as of 3:00 p.m. (CST) on January 9, 1997.
