EXCEEDING TECHNICAL SPECIFICATION L.C,O. FOR PRIMARY CONTAINMENT HPCI, AND ISOLATION CONDENSER WHILE. SHUT DOWN This PN updates PNO-III-80-234 issued onDecember 22, 1980.
The unit is returning-to power operation (critical 5: 10 a. m. CST) following the outage for scheduled maintenance and modifications.
During the outage, the reactor pressure was allowed to increase above cold shutdown (175 psig). while the Primary Containment was open, the High Pressure Coolant Injection.System.was out of service, and the Isolation Condenser was isolated.
An Immediate Action letter was issued on December 23, i980 which confirmed actions to prevent recurrence of the event.
Actions included review by the onsite Review Board; issuance of a station operating order to assure maintenance of flow and therefore adequate mixing of reactor coolant in the core; and inclusion in the_
- report of the event* the permanent changes to be. implemented to preclude recurrence of simi1ar events.
Neither the licen*see nor Region III is planning to issue a press release.
