PROCEDURE ADHERENCE PROBLEMS DURING REACTOR STARTUP DESCRIPTION:
On May 17, 2006, an operating crew at Kewaunee experienced procedure implementation issues while performing reactor startup activities to take the reactor critical following the unplanned maintenance outage that began on April 26, 2006. Information on the unplanned maintenance outage was provided in PNO-III-06-012 and PNO-III-06-012A.
The licensee determined that the operating crew performed the startup procedural steps out of sequence; however, no safety limits were exceeded and therefore the startup activity was continued. Subsequently, on the evening of May 17, 2006, the licensee had to shutdown the reactor due to equipment problems on the main turbine turning gear. The licensee initiated an event review team to determine the cause of the procedure implementation problem and to determine corrective actions.
An NRC Special Inspection Team reviewed the circumstances surrounding the event and the licensees evaluation of the cause. The licensees post event actions, including operator remediation, procedure reviews and revisions, additional training of operators prior to startup, and additional oversight by management during reactor startup activities, were determined to be adequate by the NRC team. The results of the NRCs special inspection will be issued in an inspection report in about 45 days.
The licensee adequately performed startup activities and made the reactor critical at 9:26 pm (CDT) May 22, 2006. The NRC resident inspectors were in the control room observing operator actions throughout the startup.
The State of Wisconsin will be informed. The information in this preliminary notification has been reviewed with licensee management.
