DAMAGED TROXLER PORTABLE MOISTURE DENSITY GAUGE At approximately 2:00 p.m. on August 4,1998 the Radiation Safety Officer (RS0) for the licensee notified the NRC of an event which occurred on August 4,1998 at about 3:45 a.m. The RSO stated that the event occurred at a temporary job site located on the New Jersey Turn)ike south of mile marker 60.1 in Burlington County, New Jersey. The gauge operator lad just completed a test in Lane 2 of the turnpike, returned the source to its shielded position, and walked across Lane 3 with the gauge to the median. The operator ) laced the gauge down on the ground, about three feet away, and turned his back to t1e gauge to record the results of the test. At that time a pickup .ack. ,

operated by a contractor, ran over the gauge. The gauge, a Troxler Model 3411B, j contains two sealed sources, a 5.9 millicurie cesium-137 source in the source rod drd a 39 millicurie source of americium-241 in the body of the gauge. The RSO also stated that the operator returned to the licensee's facility at about 4:45 a.m. and the RSO performed a survey of the gauge with a GM survey instrument. The RSO indicated that the source appears to be shielded but the source rod does appear to be bent. The RSO will be performing a leak test of the gauge prior to sending it to the manufacturer.

The State of New Jersey has been notified.

The Region I Office of Public Affairs is available to respond to media inquires. l Region I will dispatch an inspector to follow up on the circumstances surrounding the event.
